Heraldry Forms

Forms are in Adobe Acrobat PDF format. If you don't have Adobe Acrobat Reader, you can get it for free. If you have any questions regarding these forms, contact our Baronial Herald (use the Regnum listing). When you are ready to submit a name or device, contact our herald. There is a small fee to register a name or device.

Name Form Personal.pdf - Form to submit an SCA name. You MUST submit or already have registered an SCA name before registering a device.

Device Form Eschutcheon.pdf - The "normal" device submission form. This is a traditional shield shaped device and is the one to use under most circumstances for your first device.

Device Form Lozenge.pdf - Another option for device submission. This is a diamond shaped device and was primarily used by women. In the SCA, anyone may use it for either a first device or a later badge.

Badge Form Fieldless.pdf - Form for a "fieldless" badge. Fieldless means there is no "background" color. This can only be used for badges.

Badge Form.pdf - Another badge submission form. Use this form if you want your badge to have a field.
